GP+ Program

The GP+ Program supports smaller general practices to provide more comprehensive, multidisciplinary care by connecting patients with allied health professionals within their usual general practice. Through CESPHN commissioning, participating practices gain access to social workers and non-dispensing pharmacists who support people living with chronic and complex health conditions and those facing barriers to accessing healthcare services.

100
of participating general practices were supported to transition to the POLAR platform.
148
residential aged care homes received support to embed virtual care and telehealth into routine practice.
341
practices participated in the National Cancer Screening Register.
256
patients supported.
435
occasions of service delivered.
9
general practices participated.
22.5
of patients supported were from multicultural communities.

During 2025-26, GP+ strengthened multidisciplinary care across nine general practices in the Bayside, Georges River, Randwick and Sutherland local government areas. Social workers supported patients experiencing psychosocial challenges, including housing insecurity, financial hardship and difficulty accessing community services, while pharmacists provided medication reviews, chronic disease support and quality improvement activities.

The program supported 256 patients through 435 occasions of service, improving access to coordinated care within familiar primary care settings. Approximately 22.5% of patients supported through the program came from multicultural communities, helping reduce barriers to care and improve access to support for vulnerable populations.
